Read all of this leaflet carefully before you start taking this medicine because it contains important information for you.

1. WHAT NEBILET IS AND WHAT IT IS USED FOR
Nebilet contains nebivolol, a cardiovascular medicine belonging to the group of selective beta- blockers (i.e. with a selective effect on the cardiovascular system). This medicine lowers the heart rate and has an effect on myocardial contractility. It has a widening effect on the blood vessels, which leads to lower blood pressure.
Nebilet is used to treat increased blood pressure (hypertension).
Nebilet is also used to treat chronic heart failure in patients aged 70 or over.
2. WHAT YOU NEED TO KNOW BEFORE YOU USE NEBILET Do not take NEBILET
•   If you are allergic to nebivolol or any of the other ingredients of this medicine (listed in section 6).
•   If you have one or more of the following disorders:
-   Low blood pressure, circulation disorder in the arms or legs.
-   Very slow heart rate (less than 60 beats per minute).
-   Certain other serious heart rhythm problems (e.g. 2nd and 3rd degree atrioventricular block, heart conduction disorders).
-   Heart failure that has just occurred or has recently exacerbated, or treatment (intravenous infusion) to improve heart function.
-   Asthma or wheezing (currently or in the past).
-   Adrenal gland tumour.
-   Abnormal hepatic function.
-   Metabolic disorder (metabolic acidosis), e.g. diabetic ketoacidosis.
Warnings and precautions
Talk to your doctor if you have any of the following issues:
-   Abnormally slow heart rate.
-   A type of chest pain due to spontaneously occurring myocardial spasms, called Prinzmetal’s angina.
-   First degree heart block (a kind of mild heart conduction disorder that affects heart rhythm).
-   Poor circulation in the arms or legs, e.g. Raynaud’s disease or syndrome, cramp-like pains while walking.
-   Chronic breathing difficulties.
-   Diabetes. Nebilet has no effect on blood sugar levels, but it could conceal the warning signs of low blood sugar, such as palpitations and fast heart rate.
-   Hyperthyroidism. Nebilet may also mask the high heart rate caused by this condition.
-   Allergy: this medicine may intensify your reaction to pollen or other irritants that you are allergic to.
-   If you have or have had psoriasis (a skin disease - scaly pink patches).
-   If you are about to have surgery, always tell your anaesthetist that you are using Nebilet before receiving anesthesia.
If you have severe kidney problems, do not take this medicine for heart failure and tell your doctor.
At the beginning of the treatment, you will be monitored by a physician who is experienced in heart failure (see section 3). This treatment must not be stopped abruptly unless your doctor has so instructed (see section 3).

Children and adolescents

There is only limited data on the use of Nebilet in children and adolescents, which is why it is not recommended for use in this age group.

Other medicines and Nebilet

Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.
Certain medicines cannot be used at the same time, as they may affect the dosing requirements.
Always tell your doctor if you are using or might use any of the following medicines alongside Nebilet:
-   Medicines for high blood pressure or other heart problems, such as amiodarone, amlodipine, cibenzoline, clonidine, digoxin, diltiazem, disopyramide, felodipine, flecainide, guanfacin, hydroquinidine, lacidipine, lidocaine, methyldopa, mexiletine, moxonidine, nicardipine, nifedipine, nimodipine, nitrendipine, propafenone, quinidine, rilmenidine, verapamil.
-   Sedatives and antipsychotics (for mental illness), such as barbiturates (also used for epilepsy), phenothiazine (also used for vomiting and nausea) and thioridazine.
-   Medicines for depression, such as amitriptyline, paroxetine, fluoxetine.
-   Medicines for asthma, stuffy nose or certain eye disorders, such as glaucoma (increased pressure in the eye) or dilation of the pupil.
-   Anaesthesia medicines used during an operation.
-   Baclofen (antispasmodic substance); amifostine (protective agent used in cancer therapy).
All these medicines, including nebivolol, may influence blood pressure and heart function.
-   Medicines for excessive gastric acid, such as cimetidine - you should take Nebilet during a meal and the antacid medicine between meals.

Nebilet with food and drink

Nebilet should be taken with water and either with a meal or on an empty stomach,

Pregnancy and breast-feeding

Nebilet should not be used during pregnancy unless absolutely necessary.
Nebilet is not recommended for use while breast-feeding Talk to your doctor or pharmacist before using this medicine.

Driving and using machines

This medicine may cause tiredness. If affected, DO NOT drive or use machines.

Nebilet contains lactose

This medicine contains lactose as an excipient. If you have been told by your doctor that you have an intolerance to some sugars, talk to your doctor before taking this medicine.

3. HOW TO TAKE NEBILET

Always use this medicine exactly as described in this leaflet or as your doctor has told you.
Check with your doctor or pharmacist if you are not sure.
Nebilet may be taken during or after a meal, or independently of meals. The tablet is best
taken with some water.
Treatment of high blood pressure (hypertension)
-   The usual dose is 1 tablet per day. Preferably, the dose should be taken at the same time every day.
-   Elderly patients and patients with a kidney disorder will usually start with half a tablet a day.
-   The therapeutic effect on blood pressure becomes evident after 1-2 weeks of treatment. It may take up to 4 weeks to reach the full effect.
Chronic heart failure
-   Your treatment will be started and monitored by an experienced specialist.
-   Your doctor will begin treatment with % of a tablet (1.25 mg) a day. After 1...2 weeks, this may be increased to half a tablet (2.5 mg) a day, then to 1 tablet (5 mg) a day and then to 2 tablets (10 mg) a day until a suitable dose for you is reached. Your doctor will prescribe the dose suitable for you, follow their instructions carefully.
-   The maximum recommended dose is 2 tablets (10 mg) a day.
-   You will need to be monitored by an experienced physician for 2 hours when you start treatment and every time your dose is increased.
-   Your doctor may also reduce your dose if necessary
-   Treatment must not be stopped abruptly, as this can exacerbate your heart failure.
-   Patients with serious kidney problems must not take this medicine.
-   It is recommended to take the prescribed daily dose as a single dose, preferably at the same time every day.
If your doctor has advised you to take half or % of a tablet, follow to the instructions below on how to split the cross-scored tablets.
•   Place the tablets onto a flat, hard surface (e.g. a table) with the cross score facing up.
•   Place your index fingers on both sides of the score line and press down on the tablet (figures 1 and 2).
•   Tablet quarters can be obtained by breaking the halves in the same way (figures 3 and 4).

-   Your doctor may decide to combine different medicines to improve treatment results.
-   Do not use in children or adolescents.

If you take more Nebilet than you should

If you or somebody else has taken more tablets than prescribed, talk to your doctor immediately. The most frequent symptoms of Nebilet overdose are very slow heart rate (bradycardia), low blood pressure (hypotension) - fainting, shortness of breath such as with asthma (bronchospasm), and acute heart failure. You can take activated charcoal (over-the-counter medicine) while you wait for the arrival of the doctor.

If you forget to take Nebilet

If you forget to take the medicine, but remember a while later that you should have taken it, take your dose as usual. However, if the correct time has long passed (e.g. several hours) and the next due dose is near, skip the forgotten dose and take the usual dose at the same time the next day. Do not take a double dose. Repeated skipping should be avoided.

If you stop using Nebilet

Talk to your doctor before stopping Nebilet treatment, regardless of whether you are taking it for high blood pressure or chronic heart failure. You must not stop the treatment abruptly, as this can temporarily exacerbate your heart failure. If you need to stop Nebilet treatment for hypertension or chronic heart failure, the daily dose should be decreased gradually by halving the dose at weekly intervals.
If you have any further questions on the use of this medicine, ask your doctor or pharmacist.

4. Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them. When Nebilet is used for the treatment of high blood pressure, the possible side effects are:
Common side effects (affect less than 1 user in 10, more than 1 user in 100):
-   Headache.
-   Dizziness.
-   Tiredness.
-   Itching or tingling (paraesthesia).
-   Diarrhoea.
-   Constipation.
-   Nausea.
-   Shortness of breath.
-   Swollen hands or feet.
Uncommon side effects (affect less than 1 user in 100, more than 1 user in 1,000):
-   Slow heart rate or other heart-related complaints.
-   Overly low blood pressure.
-   Cramp-like leg pain while walking.
-   Impaired vision.
-   Impotence.
-   Depression.
-   Digestive disorders (dyspepsia), flatulence, vomiting.
-   Erythema, itching.
-   Shortness of breath such as with asthma, due to sudden cramps of the respiratory muscles (bronchospasm).
-   Nightmares.
Very rare side effects (affect less than 1 user in 10,000):
-   Fainting.
-   Exacerbation of psoriasis (a skin disease - scaly pink patches).
The following side effects have been reported only in isolated cases during Nebilet treatment: Allergic reactions influencing the whole body with generalised skin eruption (hypersensitivity).
Rapid-onset swelling around the lips and eyes or of the tongue leading to sudden breathing difficulties (angioedema).
In a clinical study for chronic heart failure, the following side effects were reported:
Very common side effects (affect more than 1 user in 10):
-   Slow heart rate.
-   Dizziness.
Common side effects (affect less than 1 user in 10, more than 1 user in 100):
-   Exacerbation of heart failure.
-   Low blood pressure (feeling faint when getting up quickly).
-   Medicine intolerance.
-   Irregular heart rhythm (first degree atrioventricular block).
-   Swelling (in the region of the ankles).
The expected benefits of using the medicine usually outweigh the risk of getting any negative side effects.

5.
HOW TO STORE NEBILET
Keep this medicine out of the sight and reach of children.
This medicine does not require any special storage conditions.
Do not use this medicine after the expiry date which is stated on the carton and blister after “Use by” or “EXP”. The expiry date refers to the last day of that month.
Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.

6. Contents of the pack and other information What Nebilet contains

- The active substance is nebivolol. Each tablet contains 5 mg of nebivolol (as nebivolol hydrochloride): 2.5 mg of d-nebivolol and 2.5 mg of l-nebivolol.
The other ingredients are: polysorbate 80 (E433), hypromellose (E464), lactose monohydrate, corn starch, croscarmellose sodium (E468), microcrystalline cellulose (E460), colloidal anhydrous silicon dioxide (E551), magnesium stearate (E572).

What Nebilet looks like and contents of the pack

The tablets are white, round and have a cross score line. The tablets are packaged in PVC/aluminium blisters. Nebilet tablets are available in packs of 7, 14, 28, 30, 50, 56, 90 or 100 tablets.
Not all pack sizes may be marketed.
