1. What Metforal is and what it is used for

Metforal contains the active substance metformin. It belongs in a group of medicines called biguanides used for the treatment of non-insulin-dependent diabetes (type 2 diabetes) in adults and children over 10 years of age.
Metforal is a medicine used for lowering high blood sugar in patients with non-insulindependent diabetes (type 2 diabetes), for whom dieting and physical exercise have not been sufficient to control blood sugar levels.
If you are overweight, taking Metforal helps to reduce your risk of complications associated with diabetes.
Adults:
Metforal may be taken on its own (monotherapy) or in combination with other medicines used for lowering blood sugar levels or insulin.
Children and adolescents:
Metforal can be used in children over 10 years of age on its own (monotherapy) or in combination with insulin.

2. What you need to know before you take Metforal Do not take Metforal

-   If you are allergic (hypersensitive) to metformin hydrochloride or any of the other ingredients of this medicine (listed in section 6).
-   If you have uncontrolled diabetes that is accompanied with severe hyperglycaemia (high blood sugar levels), nausea, vomiting, diarrhoea, rapid weight loss, lactic acidosis (see section “Risk of lactic acidosis” below) or ketoacidosis. Ketoacidosis is a condition in which substances called “ketone bodies” accumulate in the blood, which may lead to diabetic precoma. Symptoms include abdominal pain, fast and deep breathing, sleepiness or unusual fruity odour of the breath.
-   If your kidney function has decreased significantly.
-   If you have an acute medical condition that may lead to an impairment in kidney function, such as
•   dehydration due to long-lasting vomiting or severe diarrhoea;
•   severe infections;
•   circulatory disorder (shock).
Kidney problems may increase the risk of lactic acidosis (see section “Warnings and precautions” below) in the case of examinations that involve the injection of contrast media containing iodine (see section “Warnings and precautions” below).
-   If you have an acute or chronic condition which may lead to a reduced amount of oxygen in tissues
(tissue hypoxia), such as
•   heart failure or recent heart attack;
•   problems with the lungs;
•   circulatory disorder (shock).
Tissue hypoxia may increase the risk of lactic acidosis (see section “Warnings and precautions” below),
-   If you have liver failure, acute alcohol poisoning or you drink a lot of alcohol.

Warnings and precautions

Talk to your doctor before taking Metforal
-   if you have a bacterial or viral infection (e.g. the flu, respiratory infection, urinary tract infection);
-   if you take medicines that may affect kidney function (e.g. certain high blood pressure medicines, analgesics or antirheumatic medicines).
Examinations that involve the use of contrast media (see section “Do not take Metforal” above).
Tell your doctor if you are about to undergo an examination involving the injection of contrast media containing iodine. This may cause contrast medium induced nephropathy, as a result of which metformin accumulates and hence the risk of lactic acidosis increases. Your doctor will ask you to stop taking Metforal before the examination and the treatment may not continue until 48 hours have passed since the examination and once the kidney function of the patient has been re-tested and determined to be stable.
Operations
If you are about to have major surgery, you must stop taking Metforal for some time before and after the procedure. Your doctor will determine when you must stop and when you can continue taking Metforal. It is important that you carefully follow the instructions of your doctor.

Risk of lactic acidosis

Metforal may cause a very rare, but severe complication called lactic acidosis, particularly if your kidneys are not working properly. Factors that increase the risk of lactic acidosis include uncontrolled diabetes, severe infections, prolonged fasting or alcohol use, dehydration (see further information below), liver problems and any medical conditions in which there is decreased blood supply to some body part (e.g. severe heart disease).
If any of the above applies to you, ask your doctor for more information.

Stop taking Metforal for a while if you have a condition that may cause dehydration

(significant loss of body fluids), such as severe vomiting, diarrhoea, fever, exposure to heat or reduced consumption of liquids. For more information, talk to your doctor.
Stop taking Metforal and contact your doctor or the emergency department of the nearest hospital if you have any of the symptoms of lactic acidosis, as this may lead to coma.
The symptoms of lactic acidosis are:
•   Vomiting
•   Stomach pain
•   Muscle cramps.
•   Generally feeling unwell and very tired.
•   Breathing difficulties.
•   Decreased body temperature and heart rate.
Lactic acidosis is a severe medical condition that needs to be treated at the hospital.
Metforal on its own does not cause hypoglycaemia (overly low blood sugar levels). However, if you take Metforal together with other diabetes medicines that may cause hypoglycaemia (such as sulphonylureas, insulin, meglitinides), there is a risk of hypoglycaemia. If you get symptoms of hypoglycaemia, such as weakness, dizziness, increased sweating, fast heart rate, impaired vision or difficulty concentrating, it usually helps to eat or drink something containing sugar.
Metforal cannot replace the benefits of a healthy lifestyle. Continue following your doctor’s advice about dieting and get regular physical exercise.
Monitoring
•   Your doctor will regularly check your blood glucose levels and adjust your dose of Metforal accordingly. Make sure that you talk to your doctor regularly. This is particularly important in the case of children and adolescents or if you are elderly.
•   During treatment, your doctor will test your kidney function at least once a year or more frequently if you are elderly and/or your kidney function is decreasing.

Other medicines and Metforal 850 mg

If you need to get an injection of contrast media containing iodine into your bloodstream, for example, for an X-ray or scan, you must stop taking Metforal before and for the duration of the injection. Your doctor will determine when you must stop and when you can continue taking Metforal.
Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines. It may be necessary to perform more frequent blood sugar and kidney function tests or adjust the dose of Metforal. It is particularly important to tell your doctor about the following:
-   Medicines that increase the excretion of urine (diuretics).
-   Pain and inflammation medicines (NSAIDs and COX-2 inhibitors, such as ibuprofen and celecoxib).
-   Certain high blood pressure medicines (ACE inhibitors and angiotensin II receptor antagonists).
-   Medicines containing alcohol.
-   Medicines containing cortisone (corticosteroids).
-   Certain medicines used for asthma (beta-sympathomimetic medicines).
-   Ranolazine (used for treating angina) or cimetidine (used for treating digestive disorders).
In the case of long-term Metforal usage, starting or stopping treatment with other medicines may change your blood sugar levels. During this time, particular care must be taken to monitor blood sugar levels.

Metforal with food, drink, and alcohol

During treatment with Metforal, continue your diet and take special care to spread out the consumption of carbohydrates throughout the day. If you are overweight, continue your weight loss diet under medical supervision.
Avoid drinking too much alcohol while taking Metforal, as this may increase the risk of lactic acidosis (see section “Warnings and precautions”), especially if you have liver problems or are undernourished. This also applies to medicines containing alcohol.

Pregnancy, breast-feeding and fertility

If you are pregnant or breast-feeding, think you may be pregnant or are planning to have a
baby, ask your doctor or pharmacist for advice before taking this medicine.
Pregnancy
During pregnancy, you need insulin to treat your diabetes.
Breast-feeding
This medicine is not recommended while breast-feeding.
Driving and using machines
Using Metforal on its own does not affect the ability to drive or use machines.
Taking Metforal together with other blood sugar lowering medicines (such as sulphonylureas, insulin, meglitinides) may affect your ability to drive, use machines or work on unstable footing. Symptoms of hypoglycaemia include weakness, dizziness, increased sweating, rapid heart rate, impaired vision or difficulty concentrating. Do not drive or use machines if you develop these symptoms.

3. How to take Metforal

Always use this medicine exactly as your doctor has told you. Check with your doctor or pharmacist if you are not sure.
The dose of Metforal must be determined by a doctor individually for every patient depending on their blood sugar levels. Blood sugar levels must be checked regularly.
The recommended dose is:

1 film-coated tablet (equivalent to 850 mg of metformin hydrochloride).
The starting dose is 2...3 film-coated tablets (equivalent to 1,700...2,550 mg of metformin hydrochloride)
Maximum daily dose 1 film-coated tablets 3 times a day (equivalent to 2,550 mg of metformin hydrochloride).
If your kidney function has deteriorated, your doctor may decrease the dose.

The starting dose is 1 film-coated tablet (equivalent to
850 mg of metformin hydrochloride), the dose may be increased to a maximum of 2 film-coated tablets (equivalent to 1,700 mg of metformin hydrochloride).
If you are also using insulin, your doctor will tell you how to start taking Metforal.
Elderly patients
As the kidney function of elderly patients is often decreased, the dose of Metforal must be adjusted according to their kidney function.
There are tablets available that contain 500 mg or 1,000 mg of metformin hydrochloride for the individual adjustment of the dosage.

Method of administration

Swallow the tablets with or after a meal with a sufficient amount of liquid (preferably a glass of water). The score line is only there to help you break the tablet if you have difficulty swallowing it whole.
If you take two or more tablets a day, they must be spread throughout the day, for example, one tablet after breakfast and one after dinner.
If you feel that the effect of Metforal is too strong or too weak, tell your doctor.
If you take more Metforal than you should
If you have taken more Metforal than you should have, you may develop lactic acidosis. Symptoms of lactic acidosis are nausea, (lower) abdominal pain with muscle cramps, generally feeling unwell and very tired, and breathing difficulties. If these occur, you may need immediate hospital care, as lactic acidosis may lead to coma. Immediately contact your doctor or the nearest hospital for advice.
If you forget to take Metforal
Do not take a double dose to make up for a forgotten dose.
If you stop taking Metforal
Do not stop taking Metforal without talking to your doctor first.
If you stop the treatment, your blood sugar levels may increase uncontrollably and you may later develop late complications of diabetes, such as damage to the eyes, kidneys and blood vessels.
If you have any further questions on the use of this medicine, ask your doctor or pharmacist.

4. Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them.
Important side effects or symptoms that you should know about and precautions to take if they occur:
Metforal may cause a very rare (affects less than 1 user in 10,000) yet very severe side effect called lactic acidosis (see section “Warnings and precautions”). If this occurs, you must stop taking Metforal and contact your doctor or go to the emergency department of the nearest hospital, as lactic acidosis may lead to coma.

Other possible side effects:

Very common: may affect more than 1 in 10 people

-   Digestive problems, such as nausea, vomiting and diarrhoea, abdominal pain and loss of appetite. These side effects occur most often at the beginning of the treatment with Metforal and subside on their own in most cases. To prevent these symptoms, it is recommended to divide the daily dose of Metforal into two or three single doses and take them with or after a meal. If these complaints persist for a long time, stop taking Metforal and talk to your doctor.

Common: may affect up to 1 person in 10

-   Changes in the sense of taste.

Very rare: may affect up to 1 person in 10,000

-   Skin reactions, such as rash, itching and hives.
-   Abnormal results for liver function tests or inflammation of the liver (hepatitis). If you get this, stop taking Metforal and talk to your doctor.
-   Low vitamin BJ2 levels in the blood.

Side effects in children and adolescents

Limited data in children and adolescents suggests that adverse events were similar in nature and severity to those reported in adults.

How to store Metforal

Keep this medicine out of the sight and reach of children.
Do not use this medicine after the expiry date which is stated on the carton and blister. The expiry date refers to the last day of that month.
This medicine does not require any special storage conditions.
Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.

6. Contents of the pack and other information

What METFORAL contains:

The active substance is metformin hydrochloride.
One film-coated tablet contains 850 mg of metformin hydrochloride, which is equivalent to 662.9 mg of metformin.
The excipients are: hypromellose, povidone K25, magnesium stearate (Ph. Eur.) [plant-based], macrogol 6000, titanium dioxide (E171).

What METFORAL looks like and contents of the pack:

White, oblong, biconvex film-coated tablets with a score line on both sides. The blister is made from a transparent, rigid sheet of PVC which is closed with aluminium foil.
The pack contains 30, 60 or 120 film-coated tablets.
Not all pack sizes may be marketed.
