1.   What Aspirin Cardio is and what it is used for

The acetylsalicylic acid in Aspirin Cardio tablets prevents the aggregation of thrombocytes (platelets), thereby reducing the risk of thrombosis (blood clots). Aspirin Cardio tablets are thus used in the following cases:
Prophylactic treatment of coronary thrombosis (coronary artery blockage):
-   in case of suspected myocardial infarction and post-myocardial infarction;
-   in case of angina (chest pain);
-   after a coronary revascularisation procedure (restoration of blood vessels and blood flow);
-   reducing the risk of primary myocardial infarction in case of concomitant cardiovascular risk factors.
Prophylactic treatment of cerebral artery thrombosis (clot formation)
-   after a transient cerebral ischemic attack (lack of blood flow to the brain);
-   after an ischemic stroke (due to topical lack of blood flow).

2.   What you need to know before you use Aspirin Cardio Do not use Aspirin Cardio tablets:

-   if you are allergic to acetylsalicylic acid or any of the other ingredients of this medicine (listed in section 6);
-   if you have or have ever had asthma or any other allergic reactions induced by painkillers, antipyretic or anti-inflammatory drugs (salicylates or other nonsteroidal anti-inflammatory drugs);
-   if you have an acute gastrointestinal (gastric or duodenal) ulcer;
-   if you have a predilection for bleeding or haemorrhage (haemorrhagic diathesis);
-   if you have severe kidney or liver failure;
-   if you have severe heart failure
-   if you have been prescribed treatment with 15 mg or more of methotrexate (a medicine used for tumours and certain rheumatic diseases) per week (see: “Other medicines and Aspirin Cardio”);
-   if you are in the last trimester of pregnancy (see: “Pregnancy and breast-feeding”).

Warnings and precautions

Talk to your doctor or pharmacist before taking Aspirin Cardio:
-   if you are hypersensitive to analgesics, anti-inflammatory or anti-rheumatic medicines or other allergens;
-   if you have or have ever had a gastrointestinal (gastric or duodenal) ulcer, including chronic or recurring peptic ulcer disease or gastrointestinal bleeding;
-   if you have previously had episodes of gastrointestinal bleeding
-   if you are taking anticoagulants (see: “Other medicines and Aspirin Cardio”) ;
-   if you have impaired renal function or cardiovascular circulation disorders (e.g. renovascular disease, congestive heart failure, hypovolemia, major surgery, sepsis, or major bleeding), as acetylsalicylic acid can further increase the risk of renal damage and acute renal failure;
-   if you have severe glucose-6-phosphate dehydrogenase (enzyme) deficiency. Acetylsalicylic acid can cause haemolysis (rupturing of red blood cells) or haemolytic anaemia. Factors increasing the risk of haemolysis include high doses of acetylsalicylic acid, fever, and acute infections;
-   if you have impaired hepatic function;
-   if you are taking nonsteroidal anti-inflammatory drugs (used for pain, fever or inflammation) such as ibuprofen or naproxen (see: “Other medicines and Aspirin Cardio”)
-   if you have bronchial asthma, hay fever, nasal mucosal swelling (nasal polyps), chronic respiratory disease or allergies to any other substances
If you are planning to undergo surgery (including minor procedures like tooth extraction), tell your doctor or dentist that you are taking Aspirin Cardio, as it increases the risk of bleeding.
If you injure yourself, the bleeding may take longer than usual to stop. This has to do with the anticoagulant effect of Aspirin Cardio. This effect is generally insignificant in the case of very minor cuts or injuries (e.g. when shaving), but you should talk to your doctor if the bleeding is unusual or persists over a longer period of time.
Even small doses of acetylsalicylic acid reduce the excretion of uric acid. This may lead to the development of gout (a disease caused by abnormalities in the metabolism of uric acid) in predisposed patients .

Children and adolescents

Without talking to a doctor, acetylsalicylic acid is not recommended for children and adolescents under the age of 16 in case of viral infections with or without fever (incl. chickenpox, the flu), as Reye’s syndrome may occur (a life-threatening condition characterised by encephalopathy (impairment of the central nervous system), abnormal hepatic function; prolonged vomiting as the first symptom). Reye’s syndrome is a very rare but possibly life-threatening condition that requires immediate medical attention.

Other medicines and Aspirin Cardio

Tell your doctor or pharmacist if you are taking, have recently taken or might take any other medicines.
Concomitant use with Aspirin Cardio tablets (especially in larger doses) can affect the side effects of the following medicines:
Increasing the effect of the following medicines may increase the risk of side effects:
-   methotrexate (a medicine used for tumours and certain rheumatic diseases) - increases the hepatotoxicity of methotexate;
-   non-steroidal anti-inflammatory drugs (used in the case of pain, fever, and inflammation) and other rheumatic medicines - increases the risk of gastrointestinal ulcers and bleeding;
-   anticoagulants (medicines inhibiting blood coagulation), thrombolytics (medicines that dissolve blood clots) or other medicines that affect platelet (blood cells involved in coagulation) function -increases the risk of bleeding. You need to monitor yourself for any signs of external or internal bleeding (e.g. bruising) if you are taking any such medicines.
-   selective serotonin reuptake inhibitors (antidepressants) - increases the risk of gastrointestinal bleeding;
-   digoxin (heart failure medicine) - increases blood digoxin levels;
-   diabetes medicines (e.g. insulin and sulfonylurea formulations) - lowers blood sugar levels;
-   systemic glucocorticoids (oral or injectable preparations of cortisone/corticosteroids/steroids), except oral hydrocortisone used as replacement therapy in the treatment of Addison’s disease -risk of acetylsalicylic acid overdose upon discontinuation of glucocorticoid treatment
-   valproic acid (epilepsy medicine) - increases the toxicity of valproic acid
-   alcohol - increases the risk of gastrointestinal bleeding and prolongs the bleeding time
The efficacy of the following medicines may be reduced::
-   some non-steroidal anti-inflammatory drugs (e.g. ibuprofen and naproxen) - the anticoagulant and thrombolytic properties of Aspirin Cardio are reduced, making it less effective against heart attacks and strokes
-   diuretics (medicines that increase urine excretion) - decrease of glomerular filtration rate in kidneys;
-   systemic corticosteroids (oral or injectable preparations of cortisone/corticosteroids/steroids), except oral hydrocortisone used as replacement therapy in the treatment of Addison’s disease -reduced acetylsalicylic acid levels in the blood
-   angiotensin converting enzyme (ACE) inhibitors (medicines reducing blood pressure) - reduced glomerular filtration rate and blood pressure reducing effect;
-   uricosuric preparations (gout medicines that promote the excretion of uric acid, e.g. benzbromarone and probenecid) - reduced excretion of uric acid, which can cause an episode of gout.
Do not take Aspirin Cardio tablets concomitantly with the aforementioned medicines unless directed
to do so by your doctor.

Aspirin Cardio and alcohol

Aspirin Cardio tablets must not be taken with alcohol.
Pregnancy and breastfeeding
You should tell your doctor immediately if you get pregnant while taking Aspirin Cardio.
During the first and second trimester of pregnancy, Aspirin Cardio tablets can only be used under the doctor’s prescription. Aspirin Cardio tablets should not be used in the last trimester of pregnancy.
In case of regular use or large doses (over 150 mg daily), breast-feeding must be discontinued.

Driving and using machines

Aspirin Cardio does not affect the ability to drive or use machines.

3. How to use Aspirin Cardio tablets

Always use this medicine exactly as described in this leaflet or as your doctor or pharmacist has told you. Check with your doctor or pharmacist if you are not sure.
The recommended dosage for the prophylactic treatment of arterial thrombosis or for patients who have received coronary revascularisation is 1-3 tablets per day.
In case of an acute myocardial infarction, a loading dose of 100-300 mg (1-3 tablets) should be taken immediately in case of a suspicion of a myocardial infarction. The maintenance dose is 100-300 mg (1-3 tablets) per day for 30 days after the infarction. After 30 days, your doctor will determine if you require any further prophylactic treatment for myocardial infarction.
A dose of 100 mg (1 tablet) per day or 300 mg (3 tablets) every other day is used for reducing the risk of initial myocardial infarction in people with cardiovascular risk factors.
Aspirin Cardio tablet(s) should be swallowed whole, preferably 30 min before eating, with a glass of water. If Aspirin Cardio tablets are used to treat an acute myocardial infarction, the tablet(s) ingested as the loading dose should be chewed or crushed to accelerate the absorption of the medicine.

Children

Safety and efficacy data in children under the age of 18 is lacking, which is why Aspirin Cardio is not recommended to be used in children under the age of 18.

Duration of treatment

Treatment with Aspirin Cardio tablets is long term, the duration of the treatment will be determined by your doctor.
Talk to your doctor or pharmacist if you feel that the effect of Aspirin Cardio tablets is too strong or too weak.

If you take more Aspirin Cardio tablets than you should

Overdose (especially in children and the elderly) may lead to dizziness, and murmur/ringing in the ears..
If you suspect overdose, talk to your doctor immediately. Your doctor will decide on the appropriate measures to take, depending on the severity of the toxicity / overdose.

If you forgot to take Aspirin Cardio tablets

Do not take a double dose to make up for a forgotten dose. Continue using the medicine as described in this leaflet, or as your doctor or pharmacist has told you.

If you stop using Aspirin Cardio

Do not discontinue treatment without talking to your doctor.
If you have any further questions on the use of this medicine, ask your doctor or pharmacist.

4. Possible side effects

Like all medicines, this medicine can cause side effects, although not everybody gets them.
Possible effects include symptoms of gastrointestinal irritation, such as indigestion, stomach pain; in rare cases, gastrointestinal inflammation and ulcer that can, in very rare cases, cause bleeding and perforation of the gastrointestinal ulcer.
Increased risk of bleeding (bleeding, bruises, nosebleeds, bleeding from urinary conduits and genitals, and bleeding gums during or after a surgical procedure). Severe gastrointestinal or cerebral haemorrhages have occurred in rare or very rare cases, which can, in individual cases, be life-threatening.
Bleeding can cause acute or chronic anaemia / iron deficiency anaemia, the symptoms of which include fatigue and paleness.
Development of haemolysis (rupturing of red blood cells) and haemolytic anaemia has been reported in patients with severe glucose-6-phosphate hydrogenase deficiency.
Development of renal damage and acute renal failure has been reported.
Hypersensitivity reactions (asthma, rash, hives, swelling, itchiness, rhinitis, nasal congestion, changes in heart rate or rhythm, shortness of breath or breathing difficulties; in very rare cases, anaphylactic reactions, incl. anaphylactic shock, the symptoms of which include sweating, nausea, bluish-red discolouration of the skin or mucous membranes, severe loss of blood pressure, loss of consciousness, even coma, etc.) can also occur.
Very rare cases of abnormalities in hepatic function have been observed.
Dizziness and murmur in the ears have also been reported, which may be symptoms of an overdose.

5. How to store Aspirin Cardio tablets

Keep this medicine out of the sight and reach of children.
Do not store at a temperature higher than 25 °C.
Do not use this medicine after the expiry date which is stated on the carton. The expiry date refers to the last day of that month.
Do not throw away any medicines via wastewater or household waste. Ask your pharmacist how to throw away medicines you no longer use. These measures will help protect the environment.

6. Contents of the pack and other information What Aspirin Cardio contains

-   The active substance is acetylsalicylic acid. One tablet contains 100 mg of acetylsalicylic acid.
-   The excipients are powdered cellulose, corn starch, methacrylic acid-ethyl acrylate copolymer, talc, and triethyl citrate.

What Aspirin Cardio looks like and contents of the pack

White round tablets with a diameter of 7 mm.
The tablets are inside a blister pack made from clear transparent polypropylene film and aluminium sheet.
The pack contains 28 or 98 tablets.
Not all pack sizes may be marketed.
